Recovered a full day weekly and lifted margin 10% with AI system

Quick summary

A solo physician built a four-layer AI operating system in four months that recovered a full day of work weekly, increased margins 10%, and runs the entire patient journey—from discovery calls to lab intelligence—while keeping human judgment at every decision gate.

Project Description

Architecture and Alchemy: a single doctor's AI operating system. Built in 4 months inside a full clinic schedule, it lets one physician + two staff run a precision functional medicine practice at ten-person-team leverage. Four doctrine layers (Personal DNA, Business DNA, Clinical Brain, Operations Brain) feed 30+ custom skills, so every AI output inherits my voice, my business model, and my proprietary INSIGHT™ Method. AI runs the full patient journey — discovery call, chart note, lab intelligence, content — while judgment stays human at every gate.

Achievements

- Margin up 10%, team unchanged: two staff + one physician

- ~1 full day of work recovered weekly (documentation, research, content)

- Consent agent live 3+ months; INSIGHT™ charting and lab workflows now staff-run

- AI search engines now referring patients, from consistency encoded in the skills

- The system surfaced a pattern no one asked it to find: 6–7% of patients drifting quiet (AWOL) — next build flags that risk before the drift

Tools

Claude (30+ skills, Projects, Cowork scheduled runs) · Airtable · Otter.ai · Marblism consent agent · GoHighLevel · Obsidian · Google Workspace · NotebookLM

Methodology

Doctrine before tools: voice, business, clinical method, and operations documented first; every skill built on top, so nothing drifts. Procedure encoded, discernment human. HIPAA by design: chart-code de-identification, human approval at every gate. Every workflow deposits intelligence as a byproduct — calls, notes, and labs all compound. One filter for every addition: 30+ skills, not 300.

Learnings

- AI as a tool answers prompts; AI as a system holds your standards

- The bottleneck in advanced medicine is thinking capacity, not documentation

- Compliance constraints force better design when humans are placed deliberately

- AWOL marked the threshold between automation and an operating system

- A clinic can buy the architecture; it cannot buy the doctrine
